MICN Re-Authorization

• Obtain fourteen (14) hours of pre-hospital oriented continuing education and eight (8) hours of EMS Agency prescribed CE.

• Complete the application form and attach copies of:

•Current MICN Certificate

•Picture Identification, for example, California Drivers License

•Current Registered Nurse License

•Documentation of successful completion of Continuing Education

• Submit payment of the $20 reauthorization fee or click here to pay online. All requirements must be completed and all documents, fee and forms postmarked or received at the EMS Agency 15 DAYS PRIOR TO YOUR EXPIRATION DATE , in order to avoid a late fee. Please be aware that a late fee of $25.00 will be assessed to applicants who are late in submitting reauthorization material.
